During the current Coronavirus outbreak, we are unable to verify identification documents in person so here are some suggestions on how you can provide documents :-
• Download and print the registration form then complete.
Post or deliver by hand in a sealed envelope to the practice letter box, together with a copy of photographic ID, for example, driving licence or passport together with a copy of a recent utility bill (within last 3 months)
• Download the registration form and complete on screen, save ready to e-mail.
Scan the photographic ID documents, examples as stated above.
E-mail the completed registration form and ID documents to: Info.Haygarth@wales.nhs.uk
If you are unable to scan documents, you could use a mobile phone to take pictures of the documents and then e-mail.
Please note My Health On-line is not fully compatible with Internet Explorer 11. If you enter your details incorrectly, the reCaptcha box will appear for verification, however you cannot place a tick in the box to allow you to login. If you experience problems logging into My Health On-line, please use another internet browser such as Chrome or Firefox to access the website.
Due to the introduction of Total Triage, making appointments via My Health On-Line has been suspended. However, you are still be able to order your repeat prescriptions via this method and cancel appointments.
If you have access to the Internet, you can order repeat prescriptions via My Health On-line - the secure, online service.
Please note from April 2018 you will be asked to provide appropriate identification documents when registering to use this service.
You will require a registration letter from the Practice which gives you an Access ID and PIN number. If you would like to use this service, please ask a Receptionist at either Hay or Talgarth Medical Centre for a request form. Alternatively, click HERE to download the request form. The form should be completed and returned to the Medical Centre by hand with your identification documents for verification. A registration letter will then be sent to you.
This service is available to adult patients only.

If you have forgotten your log-in details and have set up security questions within your MHOL account, you can use the “forgotten details” link on the MHOL home page.
If you have forgotten your log-in details but have not set up security questions within your MHOL account, please contact the Practice – either Hay or Talgarth Medical Centre and we will arrange to reset your account key and re-verify your account to produce a new PIN document. You will be asked to complete a new request form and to provide identification documents for verification. A new registration letter will be sent to you. You will then need to re-create a MHOL account.

If we cannot help with your issue, we will advise you to use the email@example.com e-mail on the MHOL website.
The NHS Wales Informatics Service Primary Care Services Team will then assist you.
