Due to the introduction of Total Triage, making appointments via My Health On-Line has been suspended. However, you are still be able to order your repeat prescriptions via this method and cancel appointments.
If you have access to the Internet, you can order repeat prescriptions via My Health On-line - the secure, online service.
Please note from April 2018 you will be asked to provide appropriate identification documents when registering to use this service.
You will require a registration letter from the Practice which gives you an Access ID and PIN number. If you would like to use this service, please ask a Receptionist at either Hay or Talgarth Medical Centre for a request form. Alternatively, click here to download the request form. The form should be completed and returned to the Medical Centre by hand with your identification documents for verification. A registration letter will then be sent to you.
This service is available to adult patients only.

If you have forgotten your log-in details and have set up security questions within your MHOL account, you can use the “forgotten details” link on the MHOL home page.
If you have forgotten your log-in details but have not set up security questions within your MHOL account, please contact the Practice – either Hay or Talgarth Medical Centre and we will arrange to reset your account key and re-verify your account to produce a new PIN document. You will be asked to complete a new request form and to provide identification documents for verification. A new registration letter will be sent to you. You will then need to re-create a MHOL account.
